    Complex Care Solutions is requesting to have flexibility with a cred committee deadline file signature if it is signed a different date than the deadline date in order to correctly backdate when the vote was made. Currently, CCS has a process for their committees where they will run into a problem often and would like to mare for their own audit purposes that the date of the approved cred file always matches the signature date.

DELEGATION OF CREDENTIALING AGREEMENT/ADDENDUM This DELEGATION OF CREDENTIALING AGREEMENT, including any addendum attached hereto (“Agreement”) to the Vendor Master Services Agreement (“MSA”), is made and entered into as of the date of last signature by and between CCS Intervention Operations, LLC, including its affiliates (“Client”), and FirstLayerAI, Inc. d.b.a Medallion (“Delegate”) (individually referred to herein as the “Party” and collectively as the “Parties”) for the purpose of setting forth the terms pursuant to which the parties agree that Delegate shall provide certain credentialing services, as described in this Agreement and any Addendum attached hereto (“Delegated Services”) to Client. Client and Delegate are parties to that certain MSA effective August 26, 2024 (the “MSA Effective Date”). The terms of this agreement apply to all Delegated Services performed from and after the MSA Effective Date I. Mutual Agreement A.Client and Delegate agree that the following functions shall be delegated by Client to Delegate: 1.Primary source verifications as part of the delegated credentialing workflow identified in Addendum A. 2.Ongoing monitoring of providers between credentialing events. II.Assigning Responsibilities A.The specifics of what is included here can be found in Addendum A - Credentialing. Any credentialing activities not specified in this Agreement are retained by Client. B.If Delegate sub-delegates any responsibilities, Delegate will notify Client of such sub- delegation. At the time of this notification or shortly after, Delegate will provide Client with documentation to demonstrate appropriate oversight of such entity by Delegate. Client has the right to review activity performed by the sub-delegate in accordance with Delegate’s contract with the sub-delegate, which will include oversight mechanisms similar to those set forth in this Agreement. III.Reporting A.Delegate agrees to provide Client with reporting on credentialing act
