Operational pain

  • CAQH manual burden — SB manually uploading malpractice insurance, handling all reattestations, and updating demographics (e.g. Idaho location change) across all providers. Volume becomes untenable at scale (~150 malpractice uploads, 70 MT MD licenses at year-end). Exploring Medallion CAQH Management (Gong 2026-04-20).
  • Licensing over-consumption — consuming far more multi-state licenses than originally forecast; providers going to more states than planned. SB using Medallion consumption data to drive internal education with leadership on provider utilization planning (Gong 2026-04-20).
  • Aetna Montana enrollment “closed network” rejection — Frontier contracts through First Choice Health (Aetna delegates credentialing to First Choice Health in Montana); enrollment app was submitted under existing TIN but triggered a “closed network” response. Root cause: enrollment workflow needs to follow first-choice-health path (not direct Aetna); follow-up calls should go to First Choice Health, not Aetna. Leigh updating Aetna SOP (Gong 2026-04-20).
  • TriCare West / TriWest no longer issuing effective dates — aged enrollments stuck open because TriWest now provides only a reference number and confirmation of participation; will not give effective dates. HNFS → TriWest Healthcare Alliance transition also left duplicate lines (some labeled “Health Net Federal Services”). VACCN program also runs through TriWest (same administrator). Workflow fix needed: match TriCare and TriWest effective dates using same approval date from shared administrator (Gong 2026-04-20-253812143190235977).
  • Dr. Gossela compact LOQ workflow gap — LOQ was processed externally by the provider; when license requests were submitted in Medallion the LOQ was not yet approved, so licenses went to boards directly instead of through compact. Montana license submitted ~4/1; LOQ issued ~3/8. Future workflow: add a note on the request to hold submission until LOQ is provided externally (Gong 2026-04-20-253812143190235977).
