Daybreak

Snapshot

Sleep apnea MSO (dental + physician groups); launched July 2024; Medallion prospect in early evaluation — intro call 2026-04-20, demo scheduled 2026-04-24.

Profile

  • MSO (Management Service Organization) — not itself a DME supplier, physician group, or dental group; facilitates dental and physician services for sleep apnea patients under one package.
  • Launched July 2024; described as experiencing “crazy growth” as of April 2026.
  • Core offering: oral appliance (mouthpiece) as CPAP alternative for sleep apnea; dental practices are primary treatment delivery channel.
  • Works with physician groups: Beluga Health (majority of states) and iSleep (approx. 7 states, expanding); neither is a Daybreak subsidiary — Daybreak is an MSO client-relationship.
  • Dental group enrollment into medical payers is the primary Medallion use case; Medicare and some commercial payers require a dentist to file the medical claim, making in-network enrollment critical.

Operations

  • Currently doing significant out-of-network billing for the medical side due to lack of in-network dental group contracts.
  • Prior enrollment vendor: Triumph Health (Irving/Grapevine TX) — engaged for CA payer enrollments (Aetna, Cigna, UHC and others); failed to complete any of the first 5 enrollments; terminated.
  • Also used CertifyOS for physician group enrollments; CertifyOS exited the medical enrollment space (still handles dental licensing/renewals only).
  • Regence BCBS specifically requires a dentist to file the medical claim — Daybreak currently cannot serve those patients in-network.
  • Physician group (iSleep) has some existing in-network contracts; potential Medallion opportunity on medical enrollment expansion.

Active Work

Recurring cadences

  • Demo scheduled 2026-04-24 (Thursday) 11am Central; Courtney Snow attending; Van (co-founder) may join.

Open Issues & Requests

Operational pain

  • No in-network dental group contracts with medical payers → heavy out-of-network billing → limits patient access to sleep apnea treatment; direct blocker to growth.
  • Bad vendor experience with Triumph Health (0/5 CA enrollments completed; poor communication; work not progressing for 3–4 months without reason).
  • CertifyOS exited medical enrollment; physician group billing contractor reportedly working on some contracts but progress unclear.

Decisions & Commitments

  • 2026-04-20 — Zo Hooda: demo scheduled 2026-04-24 Thu 11am Central; Zo to send invite (modifiable so Van can be added) + follow-up email recap thread.
  • 2026-04-20 — Courtney Snow: primary contact for evaluation; Van (co-founder) gives final commercial approval; prefers results-based / back-end-weighted pricing given past vendor burn.
  • 2026-04-20 — Courtney Snow: top priority = accelerating enrollments (revenue acceleration > opex savings > claims denials in her stack rank).
