  • Tracy Klein credentialing at THR: incorrect tasks were auto-populated to the provider due to misconfigured task mapping; Jennifer Mikel deleted tasks and is investigating root cause of mapping to prevent recurrence (Gong 2026-04-20).
  • License expiration escalation: Tennessee license for a provider expired/went inactive due to missing Daytona police documentation; last admin note was ~1 month before expiration in February; Annie Walls requested proactive email escalation before expiration, not only admin-task notification; new service request required to proceed (Gong 2026-04-20).
